What Will You Learn?

Community health centers across the country are moving toward becoming a Patient-Centered Medical Home (PCMH). This online learning session will outline the core components of the PCMH model and the similarities and differences between the PCMH accrediting bodies. The webinar will feature leaders from two very different health care organizations that decided to apply to be accredited as a PCMH. Participants will hear why they did it and how it can be done cost-effectively and in support of Title X objectives.

After this session, participants will be able to:

  • Discuss the core components of the Patient-Centered Health/Medical Home Model.
  • Identify the three major PCMH/PCHH accrediting bodies - AAAHC, NCQA, and Joint Commission.
  • Describe the strategic motivation of two different health centers for pursuing PCMH recognition.
  • Understand the process by which those health centers (T.H.E. and North County) developed and implemented a Patient-Centered Medical Home that incorporated and supported Title X objectives.
