To make sure you understand our approach to online privacy issues, you MUST read this Privacy Policy and our Terms of Use, as a condition to using our websites.

Essential Access Health has created this privacy statement in order to demonstrate our firm commitment to the privacy of our users and visitors. The following discloses the information gathering and dissemination practices for our websites with the domain names essentialaccess.org and any other Essential Access Health website which contains a link to this Privacy Policy. If you choose not to consent to this Privacy Policy and our Terms of Use, you may not use these websites.

1. Information We Collect and How We Use It

Server Logs.

Our servers record your computer’s IP address in “server logs” when you use our websites. We use these logs to help diagnose problems with our server, and to administer our websites. Your IP address is also used to gather generalized demographic information that does not specifically identify you.

Cookies.

We may use session cookies within certain content functionality and pages of our websites. A session cookie is stored in temporary memory and is automatically deleted when the browser is closed or the client session is ended. A session cookie identifies you and/or your browser to our servers in order to enable seamless interaction with our websites, including such things as automatic login for certain site areas and/or content personalized for you. Registration, Shopping, Surveys, and Polls.

If you choose to “opt in” by signing up for our newsletters or emails, shopping at our online store, participating in a poll or survey on one of our websites, or registering for one of our conferences, workshops, or other events, these processes may require you to give us certain personal information, including your name, address, telephone number, email address, place of employment, licensing or professional credentials, and credit card or other payment information. Some information may be optional. Please note that you may not sign up, shop, complete a survey or poll, or register at our websites unless you accept our Terms of Use and Privacy Policy.

You may also have the option to send us comments or feedback following a survey, poll, or online transaction, or report any problem you may have with our websites. These actions may require or result in your giving us personally identifying information, such as your email address.

Personally identifying information such as an email address may be used to send you registration or purchase confirmation emails, as well as future Essential Access Health and related communications. Anonymous user demographic profile data may be shared with our business affiliates or advertisers on an individualized or aggregate basis. Personally identifying information may also be shared with unaffiliated third parties, though only with your prior consent.

Research Studies.

If you inquire about a research study that is being conducted by Essential Access Health, your contact information will only be used to communicate answers to your questions. If you agree to participate in a study, you will become subject to the privacy policies associated with that particular study and you will be give notice of those policies. If you choose not to participate, your contact information will be deleted from our records.

Third-Party Companies and Individuals.

In order to facilitate our business, we sometimes use third-party companies and individuals to perform functions on our behalf, including, but not limited to, registering program participants, processing credit card payments, and shipping products. We disclose personal information to third parties only to the extent necessary to charge your credit card, fill and deliver your order, complete your registration, provide customer service or credit, and send promotional announcements to you.

When you register for a workshop, conference, or other essential-access-health-sponsored event, that registration may be handled by EventBrite.com. Your registration information is collected by EventBrite and communicated to Essential Access Health in a secure and confidential manner. EventBrite has its own privacy policy, and we invite you to review that privacy policy at http://www.eventbrite.com/privacypolicy/.

When you register for webinar, that registration may be handled by WebEx. Your registration information is collected by WebEx and communicated to Essential Access Health in a secure and confidential manner. WebEx has its own privacy policy, and we invite you to review that privacy policy at http://www.cisco.com/web/siteassets/legal/privacy.html.

Notwithstanding the foregoing, third parties who provide services for us are prohibited from using your personal information for their direct marketing purposes and from sharing, selling, or otherwise using such information, unless you choose to opt in to the marketing, sharing, or other activities of the third parties involving the use of personal information.

Voluntary Contact.

If you send us information voluntarily, such as through personal correspondence or emails, or if other users or third parties send us correspondence about your activities on our sites, we may collect such information. We use such information, and other information that we obtain from your use of our website services, to resolve disputes, troubleshoot problems, and for similar purposes. Please also note that information you volunteer in public areas of the sites, such as chat rooms and bulletin boards, is not protected information, and anyone could potentially collect and use such information in ways we cannot control.

We may on occasion partner with various third party companies and may gather information regarding preferences which indicate you may be interested in “opting in” to being contacted about products or services our partners may provide. We cannot ensure that all of your private communications and other personally identifiable information will never be disclosed in ways not otherwise described in this Privacy Policy. For example, we may be forced to disclose information to the government or third parties under certain circumstances without your consent, or third parties may unlawfully intercept or access transmissions or private communications. We can (and you authorize us to) disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose us to legal liability.

For your convenience, Essential Access Health’s websites may contain links to other Internet websites. Please be aware that Essential Access Health is not responsible for the privacy practices or content of such other sites, which may differ significantly from our own. We encourage our users to be aware when they leave our sites and to read the privacy statements of each and every website that collects personally identifiable information. This Privacy Policy applies solely to information collected by Essential Access Health through its websites identified herein.

3. Security

Essential Access Health has physical, procedural and electronic security measures in place to protect against the loss, misuse and alteration of the information under our control. The servers in which we store personal information are kept in a secure environment. Only authorized personnel have access to your information. We make every effort to ensure that our websites and any third party sites we use protect your personal data using industry-standard technology (e.g. SSL encryption) to prevent unauthorized electronic access to sensitive financial information such as credit card numbers. Notwithstanding these efforts, we caution you that “perfect security” does not exist on the Internet. That’s why we also have sophisticated monitoring features on our servers that tell us if anyone was attempting to or had breached our security so we can take immediate measures against them.

4. Options / Corrections / Updates

Information provided at the time of registration or submission from a visitor who is 13 years old and older may be used for marketing and promotional purposes by Essential Access Health and our affiliated companies. To give you control over such use of your personal information and the communications directed to you, we allow you to “opt out” of the following services: receiving communications from Essential Access Health about new features or services, and receiving communications about offers from third-party companies offering a product or service that we think might be of interest to you. NOTE: You must include your First Name, Last Name, Email Address, City, State, and Zip Code in order to delete your records. Upon receipt and execution of your request for deletion, you will be sent one final notification indicating the removal of your information from our records.

5. Changes in Our Privacy Policy

We reserve the right to change this Privacy Policy at any time. If we decide to change our Privacy Policy, we will post those changes here so you will always know what information we gather, how we might use that information, whether we will disclose it to anyone, and how you can reach us to change, correct, or delete your personal information. We will also indicate at the top of this Privacy Policy the effective date of those changes. We therefore encourage you to review this Privacy Policy on an ongoing basis so you understand our current privacy practices.

6. Children Under Age 13

All Essential Access Health websites are intended only for users who are 13 years of age and older. If you are under the age of 13, you MUST exit this website now.
